How:

For all immunizations, you MUST have a doctors order to receive an immunization. Please contact your doctor to obtain an order prior to arriving. The shot will not be given without one. Your HMF doctor can also provide the order electronically.

The only document that will be accepted in lieu of payment will be a written document from your insurance company listing the exact vaccine to be given. Otherwise, you will be charged at the time of service and at that time, we will bill your insurance. Once we have received payment from your insurance company we will reimburse you. This process will take up to one month.
